Eligibility Definitions

Description of the business

This description could be a letter on the organization's letterhead, a company brochure or a screen print of your company's website. It must include:

  • Types or models of chassis upfitted by your organization
  • Description of product or service offerings, specifically identifying the extent to which your company modifies vehicle
  • Identify the company as the installer and retailer of their products

 

New

"New" is defined as any model year vehicle that was purchased or leased within the current or preceding calendar year as New.

 

Registrations

Your vehicle registration(s) must show your organization's name as the registered owner as well the following vehicle information:

  • Expiration date if your organization is tax exempt and you are not required to renew vehicle registration cards, the registration's. Expiration Date field must say "exempt" or "permanent" to be accepted.
  • VIN
  • Vehicle Model Year
  • Vehicle Make

 

Insurance cards or schedule

Proof of insurance must show the Insurance Company's insignia with your organization's name listed as the insured. The insurance documents must include the following information:

Policy number
-Policy effective and expiration dates
-Vehicle Model Year and Make (or VIN)
-If the above information is contained on multiple pages, each page should include the same policy number.

 

Lease agreements

This signed contract should include your organization's name listed as lessee and the vehicle's make and model year. (Lease Agreements are sometimes called "Equity Lease Rate Quotes").

 

Buyer Orders

This dealer form should include your organization's name listed as the purchaser, vehicle pricing, the vehicle's make and model year & VIN or Ford order number.

 

Billing Statement

This is a spreadsheet or list of vehicles on a leasing company's letterhead. Your organization's name must be listed as the customer or lessee. It must be accompanied by a signed Lease Agreement.

 

Titles

Your new vehicle's title must show your organization's name as the owner as well as the following vehicle information:

Effective or issue date (must be within 12 months of the date of enrollment)
-VIN
-Vehicle Model Year
-Vehicle Make

 

Livery License

This document must show the name and or logo of the issuing organization/department and an expiration date. The licensee or permit holder can be listed as either your organization's name or your personal name (if the Fleet Identification Number is being enrolled in your personal name).

 

"For-hire" permit

This document must show the name and or logo of the issuing organization/department and an expiration date. The licensee or permit holder can be listed as either your organization's name or your personal name (if the Fleet Identification Number is being enrolled in your personal name).

 

For Hire Insurance certificate

This certificate of insurance for hired autos can be issued to either your organization's name or your personal name (if the Fleet Identification Number is being enrolled in your personal name). It must include the following information:

  • Insurance Company name and/or logo
  • Expiration date
  • Indicator showing that the policy is for "Hired Autos" or "Hired Drivers"

 

Statement of usage intent

A brief statement on your organization's letterhead declaring that your organization is planning to purchase or lease vehicles for business use. It must be signed by an officer in the organization.

 

PUC License

This document must show the name and or logo of the issuing organization/department and an expiration date. The licensee or permit holder can be listed as either your organization's name or your personal name (if the Fleet Identification Number is being enrolled in your personal name).

 

TLC License

This document must show the name and or logo of the issuing organization/department and an expiration date. The licensee or permit holder can be listed as either your organization's name or your personal name (if the Fleet Identification Number is being enrolled in your personal name).

 

Funeral Operator's License

This document must show the name and or logo of the issuing organization/department and an expiration date. The licensee or permit holder can be listed as either your organization's name or your personal name (if the Fleet Identification Number is being enrolled in your personal name).

 

National Franchise Agreement

If your car rental company is a franchise, please provide the page or pages of your Franchise Agreement that show the name and address of your organization and signature of the authorizing parties.

 

Rental Agreement

This is a copy of the standard rental agreement that your customer's sign when renting a vehicle. It must show your organization's name, address and logo.

 

Purchase Orders

This is an internal document from your government agency requesting to procure vehicles from a dealer. It should include your organization's name as well as the following information:
- Model Year of requested vehicle(s)
- Make of requested vehicle(s)

 

Purchase Letter

This letter can come from any department of the state and it certifies that your organization is eligible to purchase vehicles off the state contract. A copy of an approval letter stating that the customer has a state grant to purchase vehicles can also be used to meet requirement. Both must be on official state letterhead.
